How Does the Customized Version Differ from the Printable PDF Version?
You’ll notice the customized version doesn’t have lines on it. I added lines initially but tested it with a few people and those who weren’t as tech savvy had trouble moving and modifying the lines.
If you are tech-savvy, you could easily add lines before printing your custom version. In addition, the font is slightly different. I wanted to be sure the font I used was free to all Canva users, whether they had a free or pro account.

How to Use the Free Printable Secret Santa Questionnaire
Using this free printable Secret Santa template is super simple! All you need to do is click the download link to access the digital files. You’ll have instant download access to both high resolution JPG and PDF file.
Next, you’ll print enough copies for everyone in your group (plus a couple extras in case someone loses theirs). Everyone should fill out their questionnaire, fold it up, and turn it in to a collection pot of some sort (a big box, bowl, etc) or to the person heading up the secret Santa gift exchange.
Lastly, have each participant pick a questionnaire out of a hat to determine who their secret Santa is. Each person will have a list of their person’s favorites, interests, dislikes, and more.
How to Use the Free Customizable Canva Secret Santa Template

- Click this link to access the free template.
- Sign In or Create an Account: You can create a free account, it does not need to be a pro account.
- Make a Copy of the Template: Once the template opens, you will see a button that says “Use Template” or “Make a copy.” This will add the template to your Canva account, so you can personalize it without affecting the original design.
- Customize the Template, including:
- Edit Text: Click on any text box to change the text. You can also modify the font, size, and color if needed.
- Add Images: Add images from Canva’s images, or you can upload your own images or choose from Canva’s library.
- Adjust Layout or Colors: You can change the layout, rearrange elements, or adjust colors by selecting and modifying the design elements.
- Save Your Work: Once you’re happy with your design, click the “Download” button at the top-right corner. Select the file format you prefer (e.g., PNG, JPG, PDF) and save it to your computer or device.
- Print: After downloading, you can print your design at home or send it to be printed at a local print shop.
How to Use the Free Google Forms Digital Secret Santa Questionnaire
Everything is going digital these days, Secret Santa included. By having everyone fill out their forms digitally, you can make it easier to keep who has who a secret. Watch the video or read the directions below for how to collect and share responses.
- Sign into your Google Account.
- Click here to the access the Google Forms template..
- Click “USE TEMPLATE” in the upper right-hand corner. This will save the template to your own Google Drive.
- Modify the form’s questions however you’d like.
- When you have finalized your form, click SEND in the upper right-hand corner.
- Select the option that works best for distributing your form (which includes entering email addresses, copying a link to share with others, or copying the code to embed online somewhere).
- After collecting the responses, you can export them by going back to your Google Form, clicking the Responses tab, and selecting the View in Sheets link or downloading the CSV file (see video).
How to Print Your Secret Santa Questionnaire
If you’re printing from home, be sure to check your print settings to get the highest quality feel. For best results, print on heavy card stock (see my favorite here) to give your Secret Santa questionnaire a polished, professional finish. If you’re printing for a group, it’s a good idea to print a few extra copies in case anyone needs a redo!
You can print it from your home printer, or for a more polished look, send it to your favorite print shop or online print shop to print on nice heavy card stock.
This Secret Santa questionnaire template is designed to print at actual size with detailed instructions for print settings and paper options. We recommend using high-quality card stock for that professional feel, but regular printer paper works just as well if you need a quick and affordable printing option.
