Free Printable Teacher Appreciation Questionnaire Gift

Get this free printable teacher appreciation questionnaire to make a sweet gift for your child’s teacher during teacher appreciation week or for the end of the school year. 

As a former teacher, I can tell you that teachers love meaningful gifts and this printable teacher questionnaire is a creative way to tell a teacher thanks. Don’t get me wrong, gift cards are always appreciated.

But there is something so special about getting a personal note from a child to say thanks. And this free about my teacher printable for kids is the perfect way to do it. 

While this was designed to be used during teacher appreciation week (or teacher appreciation day) it would honestly be one of the best ways to end the year.

Teachers put in so much hard work each year – what better way to end a great school year than to let them know how much your child enjoyed their time with their teacher. It’s a simple teacher appreciation gift idea that will truly be loved by your teacher. 

If you have the budget, I love to add a Target gift card (or gift card of choice) or small gift that is practical like classroom supplies along with the printable. So many end of year gifts are never going to be used (how many coffee mugs can one teacher have?) so I love to do something meaningful paired with something functional. 

This printable is an easy way to take a boring gift card and make it a little bit more special with this questionnaire. 

Are you a Room Parent?

If you’re a room Mom (or Dad), it can be difficult to come up with an end of year or teacher appreciation week gift that is special and that your teacher hasn’t gotten already. While they love to get school supplies or gift cards, I love to do something a little extra to go with my classroom gifts.

Print off a copy of this for each chid in your the class and ask parents to return them to use. Then, you can put all of the pages together to create a book for the teacher. It will be one of those teacher gifts that they will cherish for years to come. 

I like to send the little questionnaire in an envelope so that the teacher won’t see it. I love these manilla envelopes because then the pages won’t get bent. 

If you are a classroom assistant, paraeducator, etc. you can also do this as an activity with your students when your teacher is out of the room for something or out sick for the day. It makes the perfect gift to say thanks at the end of the year.

​Great Gift Idea for Student Teachers

It can be hard to figure out a gift for a student teacher. If you’re a teacher, this would be a great way to send your student teacher off at the end of their placement. Just like above, you just need to print off a copy for each child for them to fill out. Then, you can put them all together to make a little book. 

Bonus – it’s a great writing and fine motor activity! It would make a great activity do during the last week of school when you’re running out of things to do because grades are already turned in for the year.

Use the Questionnaire in Place of a Card!

I love to always add a card of some sort to my teacher appreciation gifts. They always add a personal touch. If you don’t have a card, print this off and have your child fill it out in place of a card. 

What Questions are on the Free Printable Questionnaire?

The free printable questionnaire focuses on the positive impact the teacher hand on the child completing it. Instead of focusing on the teacher’s favorite things, I like to focus on how the teacher has made a difference in the child’s life. 

Here is what is included:

  • Teacher’s name
  • Child’s name
  • Drawing of the teacher
  • What the teacher helped the child learn
  • The best memory the child has from the school year
  • Their favorite thing about their teacher
  • Some words to describe their teacher
  • and… what they will miss 

Your kid’s teacher will probably love the drawings of themselves the best. Kids always make the most adorable pictures and it will be so much fun for the teacher to see the cute way your child sees them.

What Age is this Questionnaire for?

It can be used with kids from a wide range of ages but is best suited for children in Elementary school. Even kids as young as kindergarten could complete this thoughtful gift with a little help. 

Can I use the Free Printable During Other Times of the Year?

I suggest only using this printable for any special occasion toward the end of the year (teacher appreciation week or day, the last day of school, a teacher’s birthday, etc). 

I don’t suggest using it during the rest of the year like the first day of school or as holiday or Christmas gifts because your child won’t be able to answer most of the questions until the end of the year.

What Type of Paper Should I use for the Questionnaire?

For the best quality possible, I suggest using white card stock to print your tags on. But, if you want to save money or don’t have card stock, it is totally okay to use regular printer paper. 

If you are making a class book for your kids’ teachers, I suggest using the cardstock so it is more durable and will last for many years.
